I accept full responsibility for my chorister’s participation as a member of the Girl Choir of South Florida along with all of the privileges and rewards that go with it. I agree to the following terms of membership:

• Membership Handbook and Policies. I understand that the Membership Handbook is available online and by request from The Girl Choir of South Florida. I have read and reviewed the Membership Handbook, including the policies within it that are referenced in this Enrollment Agreement.
• Registration and Tuition. I agree to pay the registration fee and tuition amount (subject to applicable discounts and approved tuition assistance) in accordance with the schedule and terms described in that policy. I understand that I am responsible for full payment of tuition even if I withdraw my chorister from the Girl Choir.
• Refunds. I understand that all funds paid to the Girl Choir of South Florida – including (but not limited to) fees for registration, tuition, camp, uniforms, tour, and concessions – are non-refundable.
• Attendance. I agree to ensure that my chorister attends the requisite rehearsals, performances, and other events in accordance with that policy.
• Sheet Music. I agree to care for all music assigned to my chorister. I agree to ensure that my chorister brings her music to all rehearsals and other events where music is required. I agree to pay to replace any lost or damaged music.
• Uniforms. I agree to properly care for all assigned uniform elements. I agree to have assigned uniform elements tailored to fit and to provide additional uniform elements as required. I agree to ensure that my chorister appears in a presentable uniform at all events where one is required. I agree to pay to clean, repair, or replace any soiled, damaged, or lost uniform elements assigned to me.
• Images and Recordings. I grant the Girl Choir permission to create photographic images, video recordings, audio recordings, and other likenesses of my chorister and to use those likenesses to support the activities of the Girl Choir. I understand that all such images, recordings, and likenesses are the exclusive property of the Girl Choir.
• Performances. I understand that my chorister’s participation in any performance event is at the discretion of the artistic and administrative staff of the Girl Choir.
• Volunteerism. I agree to support the activities of the Girl Choir by volunteering and participating in all fundraising activities.
• Membership. I understand that my chorister’s membership in the Girl Choir of South Florida is based upon my ability to fulfill these terms of membership. I understand that if at any point in time I fail to comply with these terms or if my chorister fails to uphold acceptable standards of rehearsal technique, performance practice, and/or attendance, the Girl Choir reserves the right to review and possibly revoke my chorister’s membership in the Girl Choir of South Florida.
